Four steps
What happens to a referral.
Step 1 · Receive
The referral starts from what the team already has
A hospital, clinic or provider starts a referral by uploading the referral PDF they already produce, or from a blank template. Drafts are saved until the team is ready to send.
- Upload a PDF or start from a template
- From hospitals, clinics and other providers

Step 2 · Standardize and triage
AI turns it into one standard referral
Amicare's AI reads the PDF into one standard structure (care needs, urgency, expected start, insurance, contacts) and flags anything missing. The team completes it and sets the urgency, so nothing is re-keyed by hand and the receiving provider knows what's pressing.
- The same structure for every referral
- Missing details flagged
- Urgency set before sending
- Reason for referral
- Post-surgical recovery, hip
- Care tasks
- Personal care, mobility support
- Hours needed
- 15 hours per week
- Urgency
- Starts within 72 h
- Preferred language
- Portuguese
- Phone number
- Not found in document
1 field needs review before this referral can be sent.
Step 3 · Route
It reaches several providers at once
The referring team sends it to the providers it recommends. Each gets an email alert and the referral in its inbox, and can accept, redirect it to partners, or decline.
- One send, several providers
- Accept, redirect or decline
- Redirects keep the referral moving

Step 4 · Close the loop
Everyone learns the outcome
When a provider accepts, the patient is emailed and the referring team sees the referral as matched, with the provider that accepted. Every step, including redirects and declines, stays in the referral's audit trail.
- Patient notified on acceptance
- Referrer sees who accepted
- A full audit trail

Three paths
Care can move on from any point.
Not every referral starts in a hospital. Amicare handles three ways a referral moves between organizations.
- Hospital or clinic to provider
A new referral
A referring team creates the referral and sends it to the providers that fit. The patient gets a secure link to follow it.
- Provider to partners
A redirect
A provider that can't take a referral sends it on to partner providers. The patient is told where it went.
- Provider to provider
An outgoing referral
A provider refers a client on to other providers. The patient gets one confirmation, not another set of choices.
Pilots
Start with a pilot.
A pilot puts one real referral pathway on Amicare, with the organizations on both sides of it.
- Step 01
Scope
Pick one referral pathway, such as discharges from a unit to home care, and the organizations on both sides of it.
- Step 02
Set up
We set up each organization's access, the providers referrals go to, and the referral form your team already uses.
- Step 03
Run and review
Send live referrals through Amicare for the pilot period, then review what changed with everyone involved.
Questions
How Amicare works
Something else? Talk to the team.
Who decides which providers receive a referral?
The referring team. They choose the providers set up on Amicare that fit, can mark the ones they prefer, and send to several at once. A provider that can't take a referral can redirect it to partners.
What does the AI do?
It reads an uploaded referral PDF and pre-fills a standard referral form, flagging required details it couldn't find. People review every referral before it's sent and make all the care decisions.
How does the referring team know what happened?
Their dashboard shows each referral's status, from sent and viewed by the patient to matched, and which provider accepted. Every step is also recorded in the referral's audit trail.
