Complete referrals, in one inbox.
For home-care agencies and rehab providers: receive standardized referrals from hospitals, clinics and partners, respond in one step, and stop re-keying intake.

Supported through institutional partnerships, innovation programs and public-sector pilots across Canada
For providers
Less time on intake. More time on care.
Intake
Complete, standardized referrals in one inbox
Referrals from hospitals, clinics and other providers arrive in one inbox in the same structure every time: care needs, urgency, requested start, address, insurance and notes. Counters show what's new, urgent and in progress.
- The same structure from every referring team
- New, urgent and in-progress at a glance
- An email alert with a direct link to each referral

Coordination
Work with partners, and send your own referrals
Message another provider about a referral, with the conversation tied to the referral rather than the patient's name. When you need to refer someone on, create an outgoing referral, pre-filled from a PDF if you have one, and send it to several providers at once.
- Provider-to-provider messages tied to a referral
- Outgoing referrals with PDF pre-fill
- A timeline for every referral you send

Decisions
Accept, redirect or decline in one step
Respond from the referral itself. Accepting emails the patient and shows the referring team who accepted. If you can't take it, redirect it to up to ten partner providers or decline, so it never stalls with you. Every decision is kept in the referral's audit trail.
- Accepting notifies the patient
- Redirect to partners in one step
- A full audit trail

See what's urgent
Counters show what's new, urgent and in progress, and each referral carries its urgency and requested start.
- Partners
Redirect, don't drop
Can't take it? Send it on to partner providers in one step, so the referral doesn't stall with you.
- Response
Decide in one step
Accept, redirect or decline from the referral itself. No phone tag to say yes or no.
- Closed loop
The patient hears back
Accepting emails the patient, and you can follow up with a proposed day and time window. The referring team sees who accepted.
- Audit
A record of every step
Each decision is logged with who made it and when.
Secure patient links
Patients open their referral through a single-purpose secure link, with an optional one-time access code.
Separate access
Referring teams, providers and networks each sign in to their own portal and see only the referrals meant for them.
Full history
Every action on a referral is recorded with the organization that took it and the time it happened.
Who it's for

Home-care agencies and rehab clinics

- Networks and franchises
Networks with several locations
A parent organization sees every referral sent to its member locations and where each one stands, without acting on their behalf.

How do referrals reach us?
Referring teams send referrals to you through Amicare. Each arrives in your inbox, and your team gets an email alert with a direct link. Signing in is required to see the referral itself.
What if we can't take a referral?
Redirect it to a partner provider so it keeps moving, or decline it. Either way the decision is recorded in the referral's audit trail, and a redirect lets the patient know where their referral went.
Can more than one provider accept the same referral?
Yes. A referral can go to several providers, and each can accept. The patient is emailed when a provider accepts, and the referring team sees who accepted.
Can our head office see what's happening across locations?
Yes. A parent organization has a read-only view of the referrals sent to each of its locations and where each one stands.
